fix: improve source name stripping with case-insensitive and domain matching

The initial implementation used exact case-sensitive matching which missed
titles like "Article - Toronto Star" when the DB source was lowercase.
Now uses case-insensitive matching, extracts publisher names from source
URLs, and has a regex fallback for common RSS title patterns.
